You can tell the staff when making the appointment or entering the exam room: "This involves my personal medical condition, and I would like to minimize the number of people present." Ask which staff members are directly involved in your care and which are there for teaching observation, and clearly state your wishes. If you also need accompanying family members to leave temporarily, you can mention that as well and ask the staff to help arrange the communication.
