Keep the qualifier "suspected" and do not rewrite it as a confirmed diagnosis. When filling in, note the original wording, date of issue, and issuing institution, and attach the referral form. If the form only allows selecting "yes" or "no," ask the receiving facility how to fill it in, and explain in the remarks that it is still to be evaluated, to be verified by the receiving physician.
