Professional answer

Contact the testing department ahead of time. Explain which position is difficult for you, how long you can usually hold it, and what assistance you might need. Ask the staff to confirm the test requirements, whether a doctor needs to evaluate you first, and whether appropriate support can be provided. Do not assume on your own that you cannot do the test. If you still have difficulty when you arrive, tell the staff again before the test starts so they can assess and arrange things.
