Patient question
For a patient with rheumatic heart disease being admitted to the hospital, how should their own daily medications be handed over?

Professional answer
Upon admission, proactively give your own medications and the original medication list to the medical staff for review. Ask which medications need to be kept by the hospital, whether you are allowed to keep any with you, and who will arrange administration during the hospitalization. Do not directly apply your pre-admission medication schedule to your hospital stay. If you are concerned that a dose was missed or not continued, ask the nurse or attending physician on the spot to verify.
