After the doctor who has been managing my hypertension on a long-term basis stops seeing patients or leaves, how do I continue my original management plan?

Contact the original department, ask who will take over follow-up and when the next available appointment is. Organize the previous treatment plan, pending items, and prescriptions that need renewal, and give them to the new doctor for review. If current medications may run out before the appointment, inform a legitimate medical facility in advance to arrange a prescription evaluation. Do not extend, reduce, or substitute medications on your own.
