Provide the date of the intervention, the hospital where it was performed, the name of the procedure, the discharge summary, information on any implants or devices, a list of current medications, any known allergies, and any post-procedure abnormalities. If documents are incomplete, contact the original hospital to obtain them; do not conceal any antiplatelet, anticoagulant, or other cardiovascular medications you are taking, and do not stop any medication on your own.
