Professional answer

Before the discharge plan is finalized, tell the attending physician, nurse, or rehabilitation team, and specifically describe the floor, steps, handrails, and the help family members can provide. Ask the team to evaluate the arrangements for returning home and daily entry and exit, and confirm whether transport support, care training, or other transitional services are needed. Do not assume that being discharged means being able to go up and down stairs independently.
